FINALFORMS REGISTRATION 2025-26
Parents/Guardians,
We are very excited to announce that the Jackson R-2 School Districts Athletic and Activities Department have partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic and activity participation forms as well as all required consent forms for your students. The most exciting news is that FinalForms saves data from year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.
You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update is made.
At this time, we ask the parents/guardians of all students planning to participate in Athletics, Band, Choir, Cheer, Spear-It Dance, Speech & Debate, Theatre and Scholar Bowl to create an account in FinalForms and register your student(s). Once you have completed this step you will then need to create your students account.
